SD Card Can Not Be Erased or Formatted on My Mac, How to Fix It

I have an SD Card that slider in turned okay, but the properties of it keep saying "read-only" permission. And in the Disk Utility, the options to erase the SD card are all disabled! The only option I can run there is "verify disk". What should I do to get the SD card functional read-and-write on Mac?
